Weekend Registered Nurse

Responsibilities:

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Completes regular re-evaluation of patient needs.
• Initiates that Plan of care and make necessary revisions as needed.
• Develops care plans, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as ordered by the medical provider.
• Counsels and provides education to the patient and family related to the patient's care needs.
• Case Management
• Prepares and updates clinical notes
• Communicates with the Medical Provider regarding the patient's needs and reports any changes in the patient's health. Review physician's orders as necessary and facilitate changes in care plan.
• Provides discharge planning as part of the care plan.
• Participates in an-call duties as defined by company policy

Position Qualifications:

• Graduate of an approved nursing program/school
• Current State of MI Licensed RN with one to two years of recent acute care experience.
• Must possess a valid driver's license and acceptable driving record
