Our culture is guided by our philosophy of Finding the Joy in the Journey, supporting both the families we serve and the team members who care for them.Responsibilities<ul><li>Complete comprehensive patient assessments and develop individualized plans of care</li></ul>Qualifications<ul><li>Current Arizona RN license (required)</li><li>Minimum 1 year of nursing experience (hospice or home health preferred)</li><li>Strong assessment, organization, and communication skills</li><li>Ability to work independently and as part of a collaborative team</li><li>Spanish-speaking preferred</li><li>Valid driver&rsquo;s license and reliable transportation</li><li>Provide skilled hospice nursing care in accordance with physician orders and regulations</li><li>Coordinate care with physicians, caregivers, and the interdisciplinary team</li><li>Educate patients and families on symptom management, disease progression, and end-of-life care</li><li>Maintain accurate, timely clinical documentation</li><li>Serve as a consistent point of contact for patients and families</li><li>Participate in on-call rotation as assigned</li></ul>Skills<ul><li>Extensive critical care experience with a strong background in hospital medicine, emergency medicine, or intensive care units (ICU).</li><li>Proven expertise in Level I Trauma Center or Level II Trauma Center environments, demonstrating ability to handle complex cases involving trauma and infectious diseases.</li><li>Experience working with pediatric (PICU), geriatric (Geriatrics), behavioral health, and medical-surgical populations within inpatient or outpatient settings.</li><li>Knowledge of physiology, diagnostic evaluation procedures, and medical terminology essential for accurate patient assessment.</li><li>Skilled in discharge planning, intake processes, utilization management, and case management practices across acute and post-acute care settings.</li><li>Familiarity with hospital records systems, medical records documentation standards, and outpatient clinic operations.</li><li>Strong communication skills for effective collaboration with multidisciplinary teams while maintaining compliance with HIPAA regulations.
