Agave Hospice & Palliative Care is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our hospice team in Arizona. Position Summary
The Hospice RN Case Manager is responsible for coordinating and providing skilled nursing care to hospice patients and their families. This role works closely with the interdisciplinary team to ensure individualized, compassionate, and reliable care throughout the hospice journey.

Responsibilities
- Complete comprehensive patient assessments and develop individualized plans of care
Qualifications
- Current Arizona RN license (required)
- Minimum 1 year of nursing experience (hospice or home health preferred)
- Strong assessment, organization, and communication skills
- Ability to work independently and as part of a collaborative team
- Spanish-speaking preferred
- Valid driver’s license and reliable transportation
- Provide skilled hospice nursing care in accordance with physician orders and regulations
- Coordinate care with physicians, caregivers, and the interdisciplinary team
- Educate patients and families on symptom management, disease progression, and end-of-life care
- Maintain accurate, timely clinical documentation
- Serve as a consistent point of contact for patients and families
- Participate in on-call rotation as assigned
Skills
- Extensive critical care experience with a strong background in hospital medicine, emergency medicine, or intensive care units (ICU).
- Proven expertise in Level I Trauma Center or Level II Trauma Center environments, demonstrating ability to handle complex cases involving trauma and infectious diseases.
- Experience working with pediatric (PICU), geriatric (Geriatrics), behavioral health, and medical-surgical populations within inpatient or outpatient settings.
- Knowledge of physiology, diagnostic evaluation procedures, and medical terminology essential for accurate patient assessment.
- Skilled in discharge planning, intake processes, utilization management, and case management practices across acute and post-acute care settings.
- Familiarity with hospital records systems, medical records documentation standards, and outpatient clinic operations.
- Strong communication skills for effective collaboration with multidisciplinary teams while maintaining compliance with HIPAA regulations. Pay: $85,000.00 - $90,000.00 per year
