Job OverviewWe are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive patient care plans, facilitate communication among multidisciplinary teams, and ensure seamless discharge planning and utilization management. Your expertise will help optimize patient outcomes, improve care experiences, and promote efficient use of healthcare resources. This position offers an exciting opportunity to make a meaningful difference in patients&rsquo; lives while working in a fast-paced, collaborative environment.Responsibilities<ul><li>Conduct thorough patient assessments, including medical history review, social needs evaluation, and care plan development.</li><li>Collaborate closely with healthcare providers, social workers, and family members to coordinate appropriate services and interventions.</li><li>Manage utilization review and utilization management processes to ensure compliance with insurance and healthcare regulations.</li><li>Facilitate discharge planning by arranging post-acute services, home health care, or hospice care as needed.</li><li>Maintain accurate documentation within EMR (Electronic Medical Record) or EHR (Electronic Health Record) systems, including progress notes and care plans.</li><li>Communicate effectively with patients and their families to provide education on treatment options, community resources, and follow-up care.</li><li>Ensure compliance with HIPAA regulations and uphold patient confidentiality at all times.</li><li>Utilize medical terminology knowledge along with CPT coding, ICD-9/10 coding to support billing and documentation processes.</li><li>Work with diverse populations across various healthcare settings such as ICU, emergency medicine, pediatrics, or trauma centers to deliver personalized case management services.</li></ul>Experience<ul><li>Strong background in critical care, acute care, or hospital medicine with familiarity in managed care environments.</li><li>Experience working with social services, behavioral health, or hospice care is advantageous.</li><li>Knowledge of medical coding systems including CPT coding and ICD classifications (ICD-9/10).</li><li>Proficiency in EMR/EHR systems and Microsoft Office applications for documentation and communication purposes.</li><li>Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.</li><li>Bilingual abilities are a plus to better serve multilingual patient populations.</li><li>Familiarity with health insurance processes, utilization review procedures, discharge planning strategies, and working within a Level II or Level I trauma center environment will set candidates apart.</li></ul>Join us as a Case Manager where your dedication directly impacts patient well-being!
