Job Overview
We are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. Responsibilities
- Conduct thorough patient assessments, including medical history review, social needs evaluation, and care plan development.
- Collaborate closely with healthcare providers, social workers, and family members to coordinate appropriate services and interventions.
- Manage utilization review and utilization management processes to ensure compliance with insurance and healthcare regulations.
- Facilitate discharge planning by arranging post-acute services, home health care, or hospice care as needed.
- Maintain accurate documentation within EMR (Electronic Medical Record) or EHR (Electronic Health Record) systems, including progress notes and care plans.
- Communicate effectively with patients and their families to provide education on treatment options, community resources, and follow-up care.
- Ensure compliance with HIPAA regulations and uphold patient confidentiality at all times.
- Utilize medical terminology knowledge along with CPT coding, ICD-9/10 coding to support billing and documentation processes.
- Work with diverse populations across various healthcare settings such as ICU, emergency medicine, pediatrics, or trauma centers to deliver personalized case management services.
Experience
- Strong background in critical care, acute care, or hospital medicine with familiarity in managed care environments.
- Experience working with social services, behavioral health, or hospice care is advantageous.
- Knowledge of medical coding systems including CPT coding and ICD classifications (ICD-9/10).
- Proficiency in EMR/EHR systems and Microsoft Office applications for documentation and communication purposes.
- Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.
- Bilingual abilities are a plus to better serve multilingual patient populations.
- Familiarity with health insurance processes, utilization review procedures, discharge planning strategies, and working within a Level II or Level I trauma center environment will set candidates apart.

Pay: $46,000.00 - $52,000.00 per year
