Our multidisciplinary team works collaboratively to provide holistic care, emphasizing empowerment, resilience, and recovery. Amethyst Wellness is currently searching for a full time service facilitator to join the team.www.amethystwellness.netResponsibilities<ul><li>Conduct thorough assessments and develop personalized treatment plans</li><li>Meet with clients anywhere from a weekly to monthly basis to provide care coordination, connection to resources, assess service needs, and check in on mental health symptoms and needs</li><li>Collaborate with CCS team members, including therapists, psychiatrists, and other community members to coordinate care</li><li>Maintain accurate and timely clinical documentation in compliance with Dane County CCS guidelines</li><li>Participate in team meetings and ongoing professional development</li><li>Promote a supportive and inclusive environment for clients and staff</li><li>Will be expected to travel to client locations, conduct sessions virtually, or utilize office location if available</li></ul>Qualifications<ul><li>Master&rsquo;s degree in Psychology, Social Work, Counseling, or related mental health field preferred</li><li>Experience working with individuals with mental illness, substance abuse, or trauma background preferred</li><li>Familiarity with Dane County CCS program standards and documentation requirements is a strong advantage</li><li>Excellent communication and interpersonal skills</li><li>Ability to work independently and as part of a multidisciplinary team</li></ul>Employment Details<ul><li>Full time position (25-30 hours per week) with flexible hours to accommodate client needs</li><li>Compensation based on experience and qualifications</li><li>Supportive work environment</li><li>Location: Dane County, Wisconsin</li></ul>To apply, please submit your resume on Indeed.Job Type: Full-timePay: $45.00 - $65.00 per hourBenefits:<ul><li>401(k)</li><li>Flexible schedule</li><li>Mileage reimbursement</li></ul>Work Location: Hybrid remote in Madison, WI 53719
