Our multidisciplinary team works collaboratively to provide holistic care, emphasizing empowerment, resilience, and recovery. Amethyst Wellness is currently searching for a full time service facilitator to join the team.

Responsibilities

• Conduct thorough assessments and develop personalized treatment plans
• Meet with clients anywhere from a weekly to monthly basis to provide care coordination, connection to resources, assess service needs, and check in on mental health symptoms and needs
• Collaborate with CCS team members, including therapists, psychiatrists, and other community members to coordinate care
• Maintain accurate and timely clinical documentation in compliance with Dane County CCS guidelines
• Participate in team meetings and ongoing professional development
• Promote a supportive and inclusive environment for clients and staff
• Will be expected to travel to client locations, conduct sessions virtually, or utilize office location if available

Qualifications

• Master’s degree in Psychology, Social Work, Counseling, or related mental health field preferred
• Experience working with individuals with mental illness, substance abuse, or trauma background preferred
• Familiarity with Dane County CCS program standards and documentation requirements is a strong advantage
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team

Employment Details

• Full time position (25-30 hours per week) with flexible hours to accommodate client needs
