Job Overview We are seeking a dedicated and experienced Home Health Administrator - Registered Nurse to oversee the delivery of high-quality patient care in a home health setting. The ideal candidate will possess strong clinical skills, leadership abilities, and a passion for providing compassionate care to patients in their homes. This role requires a comprehensive understanding of home health regulations and standards, as well as the ability to manage a team of healthcare professionals effectively.Duties<ul><li>Supervise and coordinate home health services, ensuring compliance with state and federal regulations.</li><li>Conduct patient assessments and develop individualized care plans based on clinical needs.</li><li>Oversee nursing staff and ensure adherence to best practices in patient care.</li><li>Manage patient documentation and maintain accurate medical records in compliance with HIPAA regulations.</li><li>Collaborate with interdisciplinary teams to provide comprehensive care, including case management and discharge planning.</li><li>Provide education and support to patients and families regarding health conditions, medications, and treatment plans.</li><li>Facilitate effective communication between healthcare providers, patients, and families.</li><li>Ensure quality assurance through regular audits of patient care services and adherence to NCQA standards</li><li>Maintain knowledge of current trends in home health care, including infection control protocols and emergency preparedness.</li><li>Other duties as assigned</li></ul>Qualifications<ul><li>Current Registered Nurse (RN) license in the state of practice.</li><li>Proven experience in home health care or related fields such as long-term care, assisted living, or hospice &amp; palliative medicine.</li><li>Familiarity with EMR/EHR systems such as WellSky is preferred.</li><li>Strong clinical skills including IV infusion, medication administration, patient assessment, wound care, and basic life support.</li><li>Experience with diverse patient populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus.</li><li>Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and documentation practices.</li><li>Critical thinking skills with the ability to make sound decisions under pressure; experience in acute care settings such as ICU or emergency medicine is advantageous.</li><li>Excellent communication skills with the ability to educate patients effectively on their health conditions and treatment options.</li><li>Commitment to ongoing professional development and adherence to ethical nursing practices.
