Home Health Administrator - Registered Nurse

Job Overview
We are seeking a dedicated and experienced Home Health Administrator - Registered Nurse to oversee the delivery of high-quality patient care in a home health setting. The ideal candidate will possess strong clinical skills, leadership abilities, and a passion for providing compassionate care to patients in their homes. This role requires a comprehensive understanding of home health regulations and standards, as well as the ability to manage a team of healthcare professionals effectively.

Duties

• Supervise and coordinate home health services, ensuring compliance with state and federal regulations.
• Conduct patient assessments and develop individualized care plans based on clinical needs.
• Oversee nursing staff and ensure adherence to best practices in patient care.
• Manage patient documentation and maintain accurate medical records in compliance with HIPAA regulations.
• Collaborate with interdisciplinary teams to provide comprehensive care, including case management and discharge planning.
• Provide education and support to patients and families regarding health conditions, medications, and treatment plans.
• Facilitate effective communication between healthcare providers, patients, and families.
• Ensure quality assurance through regular audits of patient care services and adherence to NCQA standards
• Maintain knowledge of current trends in home health care, including infection control protocols and emergency preparedness.
• Other duties as assigned

Qualifications

• Current Registered Nurse (RN) license in the state of practice.
• Proven experience in home health care or related fields such as long-term care, assisted living, or hospice & palliative medicine.
• Familiarity with EMR/EHR systems such as WellSky is preferred.
• Strong clinical skills including IV infusion, medication administration, patient assessment, wound care, and basic life support.
• Experience with diverse patient populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus.
• Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and documentation practices.
• Critical thinking skills with the ability to make sound decisions under pressure; experience in acute care settings such as ICU or emergency medicine is advantageous.
• Excellent communication skills with the ability to educate patients effectively on their health conditions and treatment options.
