Job Overview
We are seeking a dedicated and experienced Home Health Administrator - Registered Nurse to oversee the delivery of high-quality patient care in a home health setting. The ideal candidate will possess strong clinical skills, leadership abilities, and a passion for providing compassionate care to patients in their homes. This role requires a comprehensive understanding of home health regulations and standards, as well as the ability to manage a team of healthcare professionals effectively.
Duties
- Supervise and coordinate home health services, ensuring compliance with state and federal regulations.
- Conduct patient assessments and develop individualized care plans based on clinical needs.
- Oversee nursing staff and ensure adherence to best practices in patient care.
- Manage patient documentation and maintain accurate medical records in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to provide comprehensive care, including case management and discharge planning.
- Provide education and support to patients and families regarding health conditions, medications, and treatment plans.
- Facilitate effective communication between healthcare providers, patients, and families.
- Ensure quality assurance through regular audits of patient care services and adherence to NCQA standards
- Maintain knowledge of current trends in home health care, including infection control protocols and emergency preparedness.

Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Proven experience in home health care or related fields such as long-term care, assisted living, or hospice & palliative medicine.
- Familiarity with EMR/EHR systems such as WellSky is preferred.
- Strong clinical skills including IV infusion, medication administration, patient assessment, wound care, and basic life support.
- Experience with diverse patient populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus.
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and documentation practices.
- Critical thinking skills with the ability to make sound decisions under pressure; experience in acute care settings such as ICU or emergency medicine is advantageous.
- Excellent communication skills with the ability to educate patients effectively on their health conditions and treatment options.
