We are growing rapidly throughout California and looking for committed professionals who want to make a difference.Position Overview: We&rsquo;re seeking a dependable and organized healthcare professional, a Quality Assurance LVN, to join our team in Signal Hill.Key Responsibilities:<ul><li>Conduct routine clinical chart audits to ensure compliance with state, federal, and organizational regulations.</li><li>Review patient records for accuracy, completeness, and proper documentation by clinical staff.</li><li>Monitor adherence to quality improvement (QI) and quality assurance (QA) programs within the facility.</li><li>Identify documentation deficiencies and collaborate with staff to implement corrective actions.</li><li>Assist with regulatory surveys, inspections, and accreditation processes.</li><li>Track and report quality metrics, incident reports, and performance indicators.</li><li>Educate clinical staff on documentation standards, compliance policies, and best practices.</li><li>Participate in root cause analysis for adverse events, incidents, or patient safety concerns.</li><li>Ensure compliance with infection control protocols, patient safety policies, and clinical standards.</li><li>Maintain accurate records of quality audits, improvement initiatives, and compliance reports.</li><li>Collaborate with interdisciplinary teams to improve patient care outcomes and operational quality.</li><li>Support implementation of performance improvement plans and quality initiatives.</li></ul>Qualifications:<ul><li>Must have an associate degree (A.A.), or equivalent from a two (2) year college.</li><li>Must have at least one year's experience in home health or a related field.</li><li>Must have a current state LPN or LVN license.</li><li>Must work well with other disciplines to coordinate patient care.</li><li>Must have a current state driver's license.</li><li>Experience in reviewing nurses' notes for completeness, compliance with physicians' orders, authorized services, regulations required.</li><li>Experience in reviewing admission packets required.</li><li>Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, and C.N.A. care plan.</li><li>Effective written and verbal communication.</li></ul>Why Join Apex Infusion:<ul><li>Competitive compensation and benefits package.</li><li>Join an Inc. 5000 company, ranked 1103 among America's Fastest-Growing Private Companies and 187th in California.</li><li>Supportive leadership that fosters growth and promotes a collaborative team culture.</li><li>Be part of a compassionate, growing team committed to exceptional patient care.</li><li>We are dedicated to prioritizing the safety and well-being of both our employees and patients.</li><li>Work in a clean, modern infusion suite with a supportive clinical environment.</li><li>Opportunity to grow with our expanding California footprint.</li><li>We are designing clean, modern, and state-of-the-art infusion suites that prioritize the comfort of both employees and patients.</li></ul>Benefits:<ul><li>401(k)</li><li>Dental Insurance (Pediatric only)</li><li>Health Insurance</li><li>Paid time off</li></ul>Job Type: Full-timePay: From $40.00 per hourWork Location: In person
