Position Overview: We’re seeking a dependable and organized healthcare professional, a Quality Assurance LVN, to join our team in Signal Hill.

Key Responsibilities:

  • Conduct routine clinical chart audits to ensure compliance with state, federal, and organizational regulations.
  • Review patient records for accuracy, completeness, and proper documentation by clinical staff.
  • Monitor adherence to quality improvement (QI) and quality assurance (QA) programs within the facility.
  • Identify documentation deficiencies and collaborate with staff to implement corrective actions.
  • Assist with regulatory surveys, inspections, and accreditation processes.
  • Track and report quality metrics, incident reports, and performance indicators.
  • Educate clinical staff on documentation standards, compliance policies, and best practices.
  • Participate in root cause analysis for adverse events, incidents, or patient safety concerns.
  • Ensure compliance with infection control protocols, patient safety policies, and clinical standards.
  • Maintain accurate records of quality audits, improvement initiatives, and compliance reports.
  • Collaborate with interdisciplinary teams to improve patient care outcomes and operational quality.
  • Support implementation of performance improvement plans and quality initiatives.

Qualifications:

  • Must have an associate degree (A.A.), or equivalent from a two (2) year college.
  • Must have at least one year's experience in home health or a related field.
  • Must have a current state LPN or LVN license.
  • Must work well with other disciplines to coordinate patient care.
  • Must have a current state driver's license.
  • Experience in reviewing nurses' notes for completeness, compliance with physicians' orders, authorized services, regulations required.
  • Experience in reviewing admission packets required.
  • Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, and C.N.A. care plan.
  • Effective written and verbal communication.
