</div><div><b>Responsibilities</b><br>
</div><ul><li>Provide in-home physical therapy evaluations and treatments</li>
<li>Develop individualized plans of care based on patient needs and goals</li>
<li>Manage and grow your own caseload within our service area</li>
<li>Accurately document visits, progress notes, and discharge summaries</li>
<li>Communicate effectively with patients, families, and healthcare team members</li>
Ensure compliance with all state, federal, and agency regulations<br>

</div><ul><li>Licensed Physical Therapist (PT) in the New York state</li>
<li>Strong clinical and interpersonal skills</li>
<li>Ability to work independently and manage time effectively</li>
<li>Home care experience preferred but not required</li>
Reliable transportation and willingness to travel locally<br>
