About the Role

We are seeking a motivated and compassionate Physical Therapist (PT) to provide high-quality home care services. This role offers the unique opportunity to build and manage your own caseload while maintaining flexibility and autonomy in your schedule.

Responsibilities

Provide in-home physical therapy evaluations and treatments
Develop individualized plans of care based on patient needs and goals
Manage and grow your own caseload within our service area
Accurately document visits, progress notes, and discharge summaries
Communicate effectively with patients, families, and healthcare team members

Qualifications

Licensed Physical Therapist (PT) in the New York state
Strong clinical and interpersonal skills
Ability to work independently and manage time effectively
Home care experience preferred but not required
