Care Planning and Coordination<ul><li>Collaborate with the medical team, including physicians, physical therapists, and other healthcare professionals, to develop tailored patient care plans.</li><li>Monitor patient progress and adjust care plans to achieve the best health outcomes.</li><li>Ensure care plans reflect the patient&rsquo;s needs, preferences, and medical conditions.</li></ul>3.
