We are seeking a compassionate and dedicated Registered Nurse (RN) to join our clinician, family-owned Home Health Agency. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work independently while managing a diverse patient population.Responsibilities<ul><li>Conduct comprehensive OASIS patient assessments, including physical examinations and vital signs monitoring.</li><li>Develop and implement individualized care plans based on patient needs and preferences.</li><li>Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices.</li><li>Perform procedures such as IV infusion, catheterization, and wound care with aseptic technique.</li><li>Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services.</li><li>Educate patients and their families on health management, medication administration, and self-care techniques.</li><li>Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards.</li><li>Monitor patient progress and adjust care plans as necessary, providing critical updates to healthcare providers.</li><li>Provide support for patients with developmental disabilities, memory care needs, or those requiring hospice &amp; palliative medicine.</li></ul>Qualifications<ul><li>CURRENT AND/OR PAST HOME HEALTH EXPERIENCE REQUIRED FOR THIS POSITION</li><li>Current Registered Nurse (RN) license in NM</li><li>Proficiency with electronic medical record (EMR) systems such as Axxess is advantageous.</li><li>Familiarity with ICD coding (ICD-9/ICD-10), medical terminology, and documentation standards is essential.</li><li>Experience working with geriatric populations, or individuals requiring specialized medical interventions such as tube feeding or ostomy care is a plus.</li><li>Strong interpersonal skills with the ability to build rapport with patients from diverse backgrounds.</li><li>Commitment to ongoing professional development through training in areas like infection control, health coaching, or family planning.
