We are seeking a compassionate and dedicated Registered Nurse (RN) to join our clinician, family-owned Home Health Agency. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work independently while managing a diverse patient population.
Responsibilities
- Conduct comprehensive OASIS patient assessments, including physical examinations and vital signs monitoring.
- Develop and implement individualized care plans based on patient needs and preferences.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols and best practices.
- Perform procedures such as IV infusion, catheterization, and wound care with aseptic technique.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services.
- Educate patients and their families on health management, medication administration, and self-care techniques.
- Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards.
- Monitor patient progress and adjust care plans as necessary, providing critical updates to healthcare providers.
- Provide support for patients with developmental disabilities, memory care needs, or those requiring hospice & palliative medicine.
Qualifications
- CURRENT AND/OR PAST HOME HEALTH EXPERIENCE REQUIRED FOR THIS POSITION
- Current Registered Nurse (RN) license in NM
- Proficiency with electronic medical record (EMR) systems such as Axxess is advantageous.
- Familiarity with ICD coding (ICD-9/ICD-10), medical terminology, and documentation standards is essential.
- Experience working with geriatric populations, or individuals requiring specialized medical interventions such as tube feeding or ostomy care is a plus.
- Strong interpersonal skills with the ability to build rapport with patients from diverse backgrounds.

Job Type: Part-time
Pay: $60.00 - $150.00 per hour
Expected hours: 8 – 15 per week
