Program(s): </b>ReLeaf - Is a transitional housing program providing trauma-informed, no-cost housing and wraparound support services for survivors of domestic violence, sexual assault, human trafficking, and community violence across Southern Illinois. This program fills a critical regional gap by offering safe, flexible housing, and individualized case management to help survivors achieve long-term stability.</div><div></div><div><b><br>
Immediate Supervisor: </b>Program Manager (once hired) In the intrum, Program Director - Daniel Hyson</div><div><b><br>
Hours of Work: </b>Full-time 40 hours/ week. Crisis sitter on-call rotation.</div><div></div><div><b><br>

Case Manager </b><b>Minimum Qualifications</b>: Bachelor&rsquo;s Degree in Human Services or related field, qualifies as a Mental Health Professional (MHP). Previous case management experience is preferred.</p><div><b>
Case Manager </b><b>Special Requirements: </b>Must be 21 years of age or older to drive agency vehicle; maintains a valid driver&rsquo;s license and vehicle liability insurance; operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times; maintain First Aid &amp; CPR certification; must complete required registry clearances and criminal fingerprint background checks; schedules evening or weekend hours, as necessary; participates in crisis sitter on-call rotation.</div><p><b>
Case Manager </b><b>Skills &amp; Competencies:</b> Knowledge of local health and human services resources. Excellent active listening skills. Skilled in the delivery of case management/care coordination services. Basic crisis intervention and conflict resolution techniques. Excellent written, oral, visual, and non-verbal communication skills. Ability to work independently and collaboratively with team members. Excellent organizational skills. Excellent time management skills and the ability to meet deadlines as assigned.</p><div><b>
Case Manager j</b><b>ob duties include, but not limited to:</b></div><ul><li><div>
Provide individualized, trauma-informed case management, safety planning, and advocacy for victims of crime and their families.</div></li><li><div>
Complete intakes, assessments, and service plans that support housing stability, safety, and self-sufficiency.</div></li><li><div>
Maintain consistent client contact (minimum of 9 contacts per quarter), including weekly engagement to monitor safety, housing stability, and progress toward goals.</div></li><li><div>
Assist clients with accessing and coordinating services, including behavioral health, medical care, legal assistance, housing resources, and community supports.</div></li><li><div>
Provide or coordinate supportive services including counseling referrals, housing advocacy, employment assistance, education support, financial literacy, transportation, and life skills development.</div></li><li><div>
Support clients in securing and maintaining safe, stable housing and transitioning to permanent housing.</div></li><li><div>
Track client progress, service delivery, and outcomes in the agency&rsquo;s EHR system in a timely and accurate manner.</div></li><li><div>
Maintain complete, accurate, and audit-ready documentation in compliance with VOCA, ICJIA, and agency requirements.</div></li><li><div>
Monitor progress toward program goals and ensure service delivery aligns with required performance measures and grant objectives.</div></li><li><div>
Collaborate with program staff and community partners to ensure coordinated, wraparound care.</div></li><li><div>
Participate in team meetings, supervision, and ongoing training to support effective service delivery.</div></li><li><div>
Assist with outreach and public awareness activities as needed to promote victim services.</div></li><li><div>
Uphold trauma-informed, client-centered practices that ensure safety, confidentiality, and dignity.</div></li><li><div>
Participate in on-call rotation to support crisis sitter needs at Deaconess Union County Hospital.</div></li><li><div>
