Responsibilities may include:<ul><li>Conducting clinical assessments and developing individualized treatment plans</li><li>Providing hands-on treatment and supervising support staff as appropriate</li><li>Managing patient care, documentation, and ongoing evaluations</li><li>Collaborating with a multidisciplinary team to ensure excellent patient outcomes</li><li>Offering leadership and expertise in a supportive, clinical environment</li><li>Maintaining compliance with all state/federal guidelines and infection control standards</li></ul>Qualifications:<ul><li>Licensed in Florida as PT, OT, PTA, or COTA</li><li>1+ years of experience in home health (experience in lymphedema care is a plus)</li><li>Valid Driver&rsquo;s License</li><li>Strong computer/documentation skills</li><li>Bilingual skills are a plus</li><li>Lymphedema certification through a LANA-recognized program is a plus</li></ul>Physical Requirements:<ul><li>Ability to lift up at least 30 lbs</li><li>Ability to work from low surfaces, bend, stoop, squat during patient care</li></ul>Why This is the Right Job for You:<ul><li>Opportunity to specialize in lymphedema and other patient-centered chronic disease management programs</li><li>Professional growth within a thriving, supportive, clinician-led company</li><li>Be part of a cutting-edge home health team making a real impact on patients&rsquo; lives</li></ul>Job Type: Full-Time and Part-time positions availableReady to grow with Axiom?
