This position offers an exciting opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those recovering from illness or injury.Responsibilities<ul><li>Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices.</li><li>Monitor vital signs including blood pressure, pulse, respiration, and temperature; accurately document findings in electronic health record (EHR) systems like Epic or Cerner.</li><li>Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations and aseptic techniques to ensure patient safety.</li><li>Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, behavioral health support, and working with individuals with developmental disabilities.</li><li>Assist with medical procedures such as catheter care, phlebotomy (blood draws), and basic life support during emergencies; collaborate with healthcare teams in acute care settings like ICU or Level I Trauma Centers when necessary.</li><li>Maintain a safe environment by utilizing proper infection control practices and ensuring compliance with hospital or home health standards.</li><li>Communicate effectively with patients&rsquo; families and healthcare providers to ensure continuity of care and address any concerns promptly.</li></ul>Qualifications<ul><li>Valid CNA certification required; previous experience in home care or related settings highly preferred.</li><li>Demonstrated experience working in diverse environments such as hospitals (including ICU or emergency medicine), nursing homes, assisted living facilities, or behavioral health units.</li><li>Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate medical documentation.</li><li>Knowledge of physiology, anatomy, medical terminology, infection control practices, and aseptic techniques essential for safe patient care.</li><li>Experience supporting patients with complex needs including Alzheimer&rsquo;s disease, dementia, memory issues, or behavioral health challenges.</li><li>Ability to perform physically demanding tasks such as lifting patients using Hoyer lifts or other assistive devices; capable of standing or walking for extended periods.</li><li>Strong communication skills combined with a compassionate attitude to foster trust and comfort for clients and their families.
