Responsibilities
- Deliver direct patient care by assisting with activities of daily living such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices.
- Monitor vital signs including blood pressure, pulse, respiration, and temperature; accurately document findings in electronic health record (EHR) systems like Epic or Cerner.
- Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations and aseptic techniques to ensure patient safety.
- Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, behavioral health support, and working with individuals with developmental disabilities.
- Assist with medical procedures such as catheter care, phlebotomy (blood draws), and basic life support during emergencies; collaborate with healthcare teams in acute care settings like ICU or Level I Trauma Centers when necessary.
- Maintain a safe environment by utilizing proper infection control practices and ensuring compliance with hospital or home health standards.
- Communicate effectively with patients’ families and healthcare providers to ensure continuity of care and address any concerns promptly.
Qualifications
- Valid CNA certification required; previous experience in home care or related settings highly preferred.
- Demonstrated experience working in diverse environments such as hospitals (including ICU or emergency medicine), nursing homes, assisted living facilities, or behavioral health units.
- Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for accurate medical documentation.
- Knowledge of physiology, anatomy, medical terminology, infection control practices, and aseptic techniques essential for safe patient care.
- Experience supporting patients with complex needs including Alzheimer’s disease, dementia, memory issues, or behavioral health challenges.
- Ability to perform physically demanding tasks such as lifting patients using Hoyer lifts or other assistive devices; capable of standing or walking for extended periods.

Pay: $20.00 - $25.00 per hour
