We believe recovery is a collaborative journey and our team is committed to supporting clients every step of the way with dignity, respect, and hope.Position SummaryWe are seeking a dedicated and passionate Case Manager to join our team in Portsmouth, NH. The Case Manager plays a key role in helping clients navigate their recovery journey by providing support, education, advocacy, and coordination of services. This position is client-centered and focused on improving outcomes through relapse prevention strategies, wellness and life-skills coaching, and linkage to community supports.Key Responsibilities<ul><li>Build trusting, supportive relationships with clients, families, and community partners in a professional, empathetic manner.</li><li>Provide ongoing case management and recovery education tailored to each client&rsquo;s needs.</li><li>Facilitate or co-facilitate Behavioral Health groups focused on relapse prevention, wellness, and life skills.</li><li>Connect clients with appropriate community resources and support services to help them sustain long-term recovery.</li><li>Participate as part of an integrated care team, coordinating services and supporting treatment goals.</li><li>Maintain compliance with 42 CFR Part 2, HIPAA, and agency confidentiality standards.</li><li>Practice cultural humility and competency when engaging with clients from diverse backgrounds.</li><li>Accurately document group sessions, individual interactions, treatment plans, and case notes in a timely manner.</li><li>Participate in quality improvement initiatives and contribute to a collaborative team environment.</li><li>Support outreach and engagement efforts with local service providers and the recovery community.</li><li>Attend agency-required trainings and local/statewide professional development events.</li></ul>QualificationsMinimum Requirements:<ul><li>Valid driver&rsquo;s license and reliable transportation.</li><li>Proficiency with Microsoft Office (Outlook, Word, Excel).</li></ul>Preferred Qualifications:<ul><li>Associate&rsquo;s or Bachelor&rsquo;s degree in a related field (e.g., psychology, social work, human services).</li><li>Experience working in a substance use disorder or behavioral health setting.</li></ul>Why Join BayPoint Health<ul><li>Collaborative, team-oriented environment focused on compassionate, client-centered care.</li><li>Opportunity to make a direct and meaningful impact in the lives of individuals and families.</li><li>Ongoing professional development and training opportunities.</li></ul>Job Type: Full-timePay: $24.00 - $29.00 per hourBenefits:<ul><li>401(k)</li><li>Dental insurance</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Work Location: In person
