Schedule: Monday – Thursday, 12:00 PM – 9:00 PM
Location: BayPoint Health – Portsmouth, NH

Position Summary

We are seeking a dedicated and passionate Case Manager to join our team in Portsmouth, NH. The Case Manager plays a key role in helping clients navigate their recovery journey by providing support, education, advocacy, and coordination of services. This position is client-centered and focused on improving outcomes through relapse prevention strategies, wellness and life-skills coaching, and linkage to community supports.

Key Responsibilities

  • Build trusting, supportive relationships with clients, families, and community partners in a professional, empathetic manner.
  • Provide ongoing case management and recovery education tailored to each client’s needs.
  • Facilitate or co-facilitate Behavioral Health groups focused on relapse prevention, wellness, and life skills.
  • Connect clients with appropriate community resources and support services to help them sustain long-term recovery.
  • Participate as part of an integrated care team, coordinating services and supporting treatment goals.
  • Maintain compliance with 42 CFR Part 2, HIPAA, and agency confidentiality standards.
  • Practice cultural humility and competency when engaging with clients from diverse backgrounds.
  • Accurately document group sessions, individual interactions, treatment plans, and case notes in a timely manner.
  • Participate in quality improvement initiatives and contribute to a collaborative team environment.
  • Support outreach and engagement efforts with local service providers and the recovery community.
  • Attend agency-required trainings and local/statewide professional development events.

Qualifications

Minimum Requirements:

  • Valid driver’s license and reliable transportation.
  • Proficiency with Microsoft Office (Outlook, Word, Excel).

Preferred Qualifications:

  • Associate’s or Bachelor’s degree in a related field (e.g., psychology, social work, human services).
  • Experience working in a substance use disorder or behavioral health setting.

Job Type: Full-time

Pay: $24.00 - $29.00 per hour
