For these patients, virtual care is their only access to specialized chronic disease management.We're seeking Nurse Practitioners already credentialed with Florida Blue, Cigna, UnitedHealthcare, and/or Aetna&mdash;so you can start seeing patients immediately without credentialing delays.Position Overview100% remote, flexible contract role providing virtual chronic care management to medically complex patients across Florida.What You'll Do:<ul><li>Conduct virtual visits with patients managing multiple chronic conditions</li><li>Develop and update individualized care plans</li><li>Manage medications and coordinate care with specialists</li><li>Oversee pulmonary and cardiac rehabilitation programs</li><li>Collaborate with Respiratory Therapists, Exercise Physiologists, and Care Coordinators</li><li>Provide patient education and self-management support</li><li>Monitor patients proactively to prevent hospitalizations</li></ul>This role includes:<ul><li>Ongoing management of chronic respiratory and cardiovascular diseases</li><li>Supervision of virtual pulmonary and cardiac rehab programs</li><li>Care coordination across multiple providers</li><li>Remote patient monitoring and early intervention</li><li>Addressing social determinants of health (medication access, transportation, resources)</li></ul>Required QualificationsMust Have:<ul><li>Active Florida ARNP license (unrestricted)</li><li>Board Certification (ANCC or AANP)</li><li>Active NPI and CAQH account</li><li>Currently credentialed and in-network with at least ONE of:</li><li>Florida Blue (BCBS of Florida)</li><li>Cigna</li><li>UnitedHealthcare</li><li>Aetna</li></ul>(Credentialing with 2+ payers strongly preferred)Experience:<ul><li>Minimum 2+ years as practicing NP</li><li>Experience in chronic disease management, primary care, internal medicine, pulmonology, or cardiology</li><li>Comfortable with telehealth and supervising clinical staff remotely</li></ul>Preferred:<ul><li>Chronic care management certification (CCCP, PNP, cardiovascular, diabetes, or heart failure certification)</li><li>Experience managing patients with multiple chronic conditions</li><li>Familiarity with chronic care management billing and documentation</li></ul>Why Join Bloom Medical?Start Immediately &ndash; Already in-network?
