Bloom Medical is a telehealth provider specializing in chronic care management for patients with COPD, asthma, heart failure, hypertension, diabetes, and other chronic conditions.

We serve patients who face barriers to accessing in-person care—those in rural areas, without transportation, or with severe mobility limitations. For these patients, virtual care is their only access to specialized chronic disease management.

We're seeking Nurse Practitioners already credentialed with Florida Blue, Cigna, UnitedHealthcare, and/or Aetna—so you can start seeing patients immediately without credentialing delays.

Position Overview

100% remote, flexible contract role providing virtual chronic care management to medically complex patients across Florida.

What You'll Do:

• Conduct virtual visits with patients managing multiple chronic conditions
• Develop and update individualized care plans
• Manage medications and coordinate care with specialists
• Oversee pulmonary and cardiac rehabilitation programs
• Collaborate with Respiratory Therapists, Exercise Physiologists, and Care Coordinators
• Provide patient education and self-management support
• Monitor patients proactively to prevent hospitalizations

This role includes:

• Ongoing management of chronic respiratory and cardiovascular diseases
• Supervision of virtual pulmonary and cardiac rehab programs
• Care coordination across multiple providers
• Remote patient monitoring and early intervention
• Addressing social determinants of health (medication access, transportation, resources)

Required Qualifications

Must Have:

• Active Florida ARNP license (unrestricted)
• Board Certification (ANCC or AANP)
• Active NPI and CAQH account
• Currently credentialed and in-network with at least ONE of:
• Florida Blue (BCBS of Florida)
• Cigna
• UnitedHealthcare
• Aetna

(Credentialing with 2+ payers strongly preferred)

Experience:

• Minimum 2+ years as practicing NP
• Experience in chronic disease management, primary care, internal medicine, pulmonology, or cardiology
• Comfortable with telehealth and supervising clinical staff remotely

Preferred:

• Chronic care management certification (CCCP, PNP, cardiovascular, diabetes, or heart failure certification)
• Experience managing patients with multiple chronic conditions
• Familiarity with chronic care management billing and documentation

Our patients typically have:

• 3+ chronic conditions (COPD, heart failure, diabetes, hypertension)
• History of multiple hospitalizations
• Geographic isolation (rural areas) or mobility impairments
• Complex medication regimens (10+ medications)
• Limited access to specialists or care coordination

Your work prevents hospitalizations, improves quality of life, and provides coordinated care to patients who would otherwise fall through the cracks.
