For over 34 years, we&rsquo;ve provided critical financial assistance and support to families in need.We&rsquo;re seeking a Program Manager &ndash; Family Support &amp; Grants to coordinate funding programs, manage grant distribution, and serve as a key point of contact for families and partners.Work Environment<ul><li>Hybrid: Work from home with some in-person responsibilities</li><li>Home visits in Nassau &amp; Suffolk County, NY</li><li>Equipment provided for remote work</li><li>Participation in fundraising events and community outreach</li></ul>Key Responsibilities<ul><li>Serve as primary contact for families, care managers, and agencies regarding funding requests</li><li>Guide applicants through available funding programs and application processes</li><li>Coordinate and process grant and reimbursement requests, including Review Committee preparation</li><li>Submit required trackers to OPWDD and ensure compliance with all regulations</li><li>Manage fund distribution (reimbursements and direct payments) and maintain accurate reports</li><li>Complete and submit all required reports to public and private grantors</li><li>Maintain ongoing communication with families regarding funding status</li><li>Track and update client and program data</li><li>Assist with fundraising events, outreach, and community engagement</li><li>Attend meetings and support program operations as needed</li></ul>Qualifications<ul><li>Bachelor&rsquo;s degree preferred</li><li>NYS OPWDD experience strongly preferred</li><li>Background in healthcare, social services, or nonprofit programs</li><li>Bilingual (Spanish) preferred</li><li>Strong organizational, communication, and problem-solving skills</li><li>Proficient in Microsoft Word, Excel, and Teams</li><li>Valid driver&rsquo;s license and ability to travel locally</li><li>Ability to lift up to 50 lbs</li></ul>Salary$50,000 &ndash; $65,000 annually, with potential for higher compensation based on relevant experience (OPWDD, healthcare, or nonprofit program management)Why Join Us<ul><li>Established, family-founded nonprofit with a strong mission</li><li>Flexible hybrid work environment</li><li>Opportunity to make a direct impact on families in your community</li></ul>- Note: This is not an exhaustive list of responsibilities and requirements.
