We’re seeking a Program Manager – Family Support & Grants to coordinate funding programs, manage grant distribution, and serve as a key point of contact for families and partners.

Work Environment

  • Hybrid: Work from home with some in-person responsibilities
  • Home visits in Nassau & Suffolk County, NY
  • Equipment provided for remote work
  • Participation in fundraising events and community outreach

Key Responsibilities

  • Serve as primary contact for families, care managers, and agencies regarding funding requests
  • Guide applicants through available funding programs and application processes
  • Coordinate and process grant and reimbursement requests, including Review Committee preparation
  • Submit required trackers to OPWDD and ensure compliance with all regulations
  • Manage fund distribution (reimbursements and direct payments) and maintain accurate reports
  • Complete and submit all required reports to public and private grantors
  • Maintain ongoing communication with families regarding funding status
  • Track and update client and program data
  • Assist with fundraising events, outreach, and community engagement
  • Attend meetings and support program operations as needed

Qualifications

  • Bachelor’s degree preferred
  • NYS OPWDD experience strongly preferred
  • Background in healthcare, social services, or nonprofit programs
  • Bilingual (Spanish) preferred
  • Strong organizational, communication, and problem-solving skills
  • Proficient in Microsoft Word, Excel, and Teams
  • Valid driver’s license and ability to travel locally
  • Ability to lift up to 50 lbs

Salary

$50,000 – $65,000 annually, with potential for higher compensation based on relevant experience (OPWDD, healthcare, or nonprofit program management)

Job Type: Full-time

Pay: $24.50 - $31.25 per hour
