⭐ Your MissionTo deliver skilled nursing services AND design, launch, and scale a statewide Home Health division - building the systems, workflows, team, and payer relationships needed to create a financially strong program with exceptional clinical quality.You are not inheriting a system. You are building the system.You will begin hands-on and grow into the role of General Manager of Home Health Services as the division becomes profitable.⭐ What You Will Lead - AND Do1. Build &amp; Launch (Day 1)<ul><li>Lead the clinical launch of Bridging Care&rsquo;s licensed Home Health agency under WAC 246-335</li><li>Architect clinical workflows: referral acceptance &rarr; SOC assessment &rarr; Plan of Care &rarr; skilled visits &rarr; documentation &rarr; billing &rarr; QA</li><li>Create documentation templates, clinical standards, and compliance protocols from scratch</li><li>Support MCO contracting (CHPW, CCW, Molina, UHC, Kaiser, Wellpoint)</li><li>Integrate Home Health services seamlessly with Bridging Care&rsquo;s 2,200-member care coordination model</li><li>Support referral acceptance decisions from a clinical appropriateness and safety perspective</li></ul>2. Provide Clinical Services (Bootstrap Phase)To keep the division lean and self-funding, you will:<ul><li>Conduct Start of Care (SOC) assessments, initial evaluations, and skilled nursing visits</li><li>Develop and implement individualized Plans of Care</li><li>Coordinate interdisciplinary services across skilled disciplines (RN, PT, OT, ST, HHA)</li><li>Maintain excellent documentation and compliance with WAC 246-335 standards</li><li>Begin generating revenue immediately while building the division</li></ul>This phase lasts only until the program becomes profitable - then your caseload decreases as leadership expands.3. Grow &amp; Scale (Months 4&ndash;12)<ul><li>Hire and supervise additional skilled clinicians - RNs, PTs, OTs, STs, and HHAs (initially 1099, then W2)</li><li>Build an integrated, multidisciplinary clinical team across Pierce County and South King County</li><li>Implement dashboards, KPIs, and scalable operational processes</li><li>Develop productivity, quality, and financial performance systems</li></ul>4. A typical week includes:<ul><li>In-home and community field visits &mdash; you go where your members are, in their homes and at their clinics, building the kind of relationship no clinic appointment allows</li><li>Health Action Plan development &mdash; with each member, you build a personalized plan addressing chronic conditions, goals, and the real-world barriers between them and better health</li><li>Cross-provider coordination &mdash; you are the connective tissue between the member's doctors, behavioral health providers, housing supports, and social services</li><li>Telehealth visits and phone outreach &mdash; Tier 2 and Tier 3 visit types in addition to in-person field work</li><li>Accurate, timely documentation in OneHAP and Lead Organization systems &mdash; we train you completely on every system before you see your first member</li></ul>Backgrounds We Specifically Want to Hear From<ul><li>Community Health Workers (CHWs) &mdash; especially those with WA DOH Core Competency certification. Your credibility in the community is a genuine asset.</li><li>Housing navigators and case aide workers &mdash; if you have been doing home visits, case files, and resource navigation in any social services setting for 3+ years, your experience translates directly.</li><li>Bilingual and bicultural community advocates &mdash; Spanish, Somali, Vietnamese, Amharic, and other language communities are heavily represented in our member population. Bilingual CCs are exceptionally high-value and in short supply.</li><li>Recovery coaches and substance use outreach workers &mdash; your lived experience and community trust are not entry-level.
