In this role, you will provide vital patient care and support under the supervision of nursing staff, ensuring a safe, comfortable, and respectful environment for all patients. Your energy, attention to detail, and commitment to excellence will help us deliver outstanding care across various settings, including long-term care, acute care, and community-based services.Responsibilities<ul><li>Assist with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.</li><li>Monitor and record vital signs accurately, including blood pressure, temperature, pulse, and respiration rates.</li><li>Support patient mobility and safety by implementing proper transfer techniques and ensuring a safe environment.</li><li>Document patient information promptly and accurately in electronic health records (EHR) systems such as Epic or Cerner while maintaining HIPAA compliance.</li><li>Administer medications and treatments as directed by licensed nursing staff following aseptic techniques and safety protocols.</li><li>Provide compassionate emotional support to patients with diverse needs, including those with dementia, Alzheimer's disease, or behavioral health conditions.</li><li>Collaborate effectively with multidisciplinary teams to coordinate patient care plans in inpatient, outpatient, or home care settings.</li></ul>Skills<ul><li>Proven experience working with individuals with disabilities or in senior care environments such as assisted living or nursing homes.</li><li>Familiarity with EMR/EHR systems like Epic or Cerner for medical documentation and patient records management.</li><li>Knowledge of medical terminology, anatomy, physiology, and vital signs assessment techniques.</li><li>Ability to perform phlebotomy procedures and catheter care when required.</li><li>Strong understanding of infection control practices including aseptic technique and infectious disease care protocols.</li><li>Experience in acute care settings such as ICU or Level I Trauma Centers is highly desirable.</li><li>Excellent caregiving skills with a focus on geriatrics, dementia care, memory care, behavioral health, and post-acute services.</li><li>Ability to operate medical equipment like Hoyer lifts safely and efficiently.</li><li>Effective communication skills for working with children, adults, families, and healthcare teams in diverse environments such as hospitals or community clinics.
