Key Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
- Support mobility, transfers, and positioning to ensure safety
- Measure and document vital signs (blood pressure, pulse, temperature, respiration) as directed
- Assist with basic clinical support tasks within scope (per training and care plan)
- Observe, document, and report changes in patient condition
- Provide companionship and emotional support
- Maintain a clean, safe, and respectful home environment
- Familiarity with HIPAA regulations to ensure patient confidentiality.
- Follow individualized care plans, infection-control standards, and agency policies
Schedule Options
Part-time or PRN / On-call
Shifts: 6am–2pm | 1pm - 6pm
Days Shift only, Weekends
Typical shift: 4+ hours (1 client per shift)
Qualifications
- High school diploma or equivalent
- PCT certification or completion of a recognized patient care training program
- CNA certification strongly preferred (or equivalent experience)
- CPR/First Aid certification (or willingness to obtain)
- Reliable transportation and ability to travel to client homes
- Ability to pass background check, health screening, and competency evaluation

- Flexibility to work various shifts as needed.

Responsibilities
- Assist with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary.
- Monitor and record vital signs accurately, including blood pressure, temperature, pulse, and respiration rates.
- Support patient mobility and safety by implementing proper transfer techniques and ensuring a safe environment.
- Document patient information promptly and accurately in electronic health records (EHR) systems such as Epic or Cerner while maintaining HIPAA compliance.
- Administer medications and treatments as directed by licensed nursing staff following aseptic techniques and safety protocols.
- Provide compassionate emotional support to patients with diverse needs, including those with dementia, Alzheimer's disease, or behavioral health conditions.
- Collaborate effectively with multidisciplinary teams to coordinate patient care plans in inpatient, outpatient, or home care settings.
Skills
- Proven experience working with individuals with disabilities or in senior care environments such as assisted living or nursing homes.
- Familiarity with EMR/EHR systems like Epic or Cerner for medical documentation and patient records management.
- Knowledge of medical terminology, anatomy, physiology, and vital signs assessment techniques.
- Ability to perform phlebotomy procedures and catheter care when required.
- Strong understanding of infection control practices including aseptic technique and infectious disease care protocols.
- Experience in acute care settings such as ICU or Level I Trauma Centers is highly desirable.
- Excellent caregiving skills with a focus on geriatrics, dementia care, memory care, behavioral health, and post-acute services.
- Ability to operate medical equipment like Hoyer lifts safely and efficiently.
- Effective communication skills for working with children, adults, families, and healthcare teams in diverse environments such as hospitals or community clinics.
