Chino Hills Adult Day Health Care is currently looking for a social worker, preferably Master's in Social Work (MSW), or a master's in related field (Gerontology, MFT) and a social worker assistant with BA in Social Worker or related filed.Duties: (include but not limited to) Complete initial evaluations, reassessments, quarterly assessments, home assessments/visits, and flowsheets. Provide weekly support group counseling, case management, and discharge planning. Work closely with the LCSW providing counseling and to participants with mental health needs and more.Required Education: Master's in Social work/related field (gerontology); Bachelor's in Social work/related field (gerontology, Psychology)Required Language: Fluent in English/Mandarin ChineseM-F 8:00 a.m.-3:30 p.m.Salary will depend on experience/education.Job Type: Full-timePay: $25.00 - $30.00 per hourBenefits:<ul><li>Dental insurance</li><li>Health insurance</li><li>Vision insurance</li></ul>Work Location: In person
