Social Worker/Social Worker Assistant (Mandarin & English)

Chino Hills Adult Day Health Care is currently looking for a social worker, preferably Master's in Social Work (MSW), or a master's in related field (Gerontology, MFT) and a social worker assistant with BA in Social Worker or related filed.

Duties: (include but not limited to) Complete initial evaluations, reassessments, quarterly assessments, home assessments/visits, and flowsheets. Provide weekly support group counseling, case management, and discharge planning. Work closely with the LCSW providing counseling and to participants with mental health needs and more.

Required Education: Master's in Social work/related field (gerontology); Bachelor's in Social work/related field (gerontology, Psychology)

Required Language: Fluent in English/Mandarin Chinese

M-F 8:00 a.m.-3:30 p.m.

Salary will depend on experience/education.

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
