Position Summary: 
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home health team. This is a rewarding opportunity to deliver high-quality, one-on-one care in a home setting. 
Key Responsibilities:<ul><li>
Provide skilled nursing visits in patients&rsquo; homes (wound care, med management, assessments, etc.)</li><li>
Communicate effectively with care team and patients</li><li>
Maintain accurate clinical documentation</li><li>
Promote patient independence and well-being</li></ul>

Qualifications:<ul><li>
Current RN license in PA</li><li>
Minimum 1 year of nursing experience (Home Health experience preferred)</li><li>
Reliable transportation and valid driver&rsquo;s license</li><li>
Compassionate, dependable, and a good communicator</li></ul>
