Position Summary:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home health team. This is a rewarding opportunity to deliver high-quality, one-on-one care in a home setting.

Key Responsibilities:

• Provide skilled nursing visits in patients’ homes (wound care, med management, assessments, etc.)

• Communicate effectively with care team and patients

• Maintain accurate clinical documentation

• Promote patient independence and well-being

Qualifications:

• Current RN license in PA

• Minimum 1 year of nursing experience (Home Health experience preferred)

• Reliable transportation and valid driver’s license

• Compassionate, dependable, and a good communicator
