This position will be responsible for Moreno Valley.We are seeking a motivated and compassionate Case Manager to join our team and work with young adults aged 18 to 21. The Case Manager will play a pivotal role in providing individualized support, guidance, and resources to help our clients achieve independence and success.<ul><li>Conduct thorough assessments of young adults' needs, strengths, and goals.</li><li>Develop and implement personalized case plans in collaboration with clients.</li><li>Provide guidance and support in areas such as education, employment, housing, and personal development.</li><li>Connect clients to relevant community resources and services.</li><li>Monitor progress and adjust case plans as needed.</li><li>Advocate for clients' rights and interests.</li><li>Offer crisis intervention and emotional support as required.</li><li>Maintain accurate and up-to-date case records.</li><li>Collaborate with a multidisciplinary team of professionals, including social workers, counselors, and educators.</li><li>Participate in regular weekly team meetings and professional development opportunities.</li><li>Case Manager MUST Assist with client move in and move outs</li></ul>Case Manager Qualifications:<ul><li>Bachelor's degree in social work, psychology, counseling, or a related field</li><li>Previous experience working with young adults in a case management capacity.</li><li>Strong interpersonal and communication skills.</li><li>Empathetic and client-centered approach to service delivery.</li><li>Knowledge of local resources and support services for young adults.</li><li>Ability to work independently and as part of a team.</li><li>Excellent organizational and documentation skills.</li><li>Cultural competence and an understanding of the unique challenges facing young adults.</li></ul>Case Manager Benefits:<ul><li>Competitive salary commensurate with experience.</li><li>Dental, and vision insurance options.</li><li>Retirement savings plan.</li><li>Generous paid time off and holidays.</li><li>Ongoing professional development opportunities.</li><li>A positive and inclusive work environment dedicated to making a difference in the lives of young adults.</li></ul>Interested candidates are encouraged to submit a resume, cover letter should include your current &amp; past experience to help the youth, and three professional references to resumes@downsmartin.org.
