Case Manager/ Life Coach - Young Adult, Transitional Housing Exp Required

DMCS Transitional Housing is a leading nonprofit organization dedicated to empowering and supporting young adults as they navigate the challenges of transitioning into adulthood. We provide a range of services and resources to help young adults build essential life skills, overcome obstacles, and achieve their goals. Our team is passionate about making a positive impact on the lives of the next generation. This position will be responsible for Moreno Valley.

We are seeking a motivated and compassionate Case Manager to join our team and work with young adults aged 18 to 21. The Case Manager will play a pivotal role in providing individualized support, guidance, and resources to help our clients achieve independence and success.

• Conduct thorough assessments of young adults' needs, strengths, and goals.
• Develop and implement personalized case plans in collaboration with clients.
• Provide guidance and support in areas such as education, employment, housing, and personal development.
• Connect clients to relevant community resources and services.
• Monitor progress and adjust case plans as needed.
• Advocate for clients' rights and interests.
• Offer crisis intervention and emotional support as required.
• Maintain accurate and up-to-date case records.
• Collaborate with a multidisciplinary team of professionals, including social workers, counselors, and educators.
• Participate in regular weekly team meetings and professional development opportunities.
• Case Manager MUST Assist with client move in and move outs

Case Manager Qualifications:

• Bachelor's degree in social work, psychology, counseling, or a related field
• Previous experience working with young adults in a case management capacity.
• Strong interpersonal and communication skills.
• Empathetic and client-centered approach to service delivery.
• Knowledge of local resources and support services for young adults.
• Ability to work independently and as part of a team.
• Excellent organizational and documentation skills.
• Cultural competence and an understanding of the unique challenges facing young adults.
