Job Overview We are seeking a highly motivated and detail-oriented MDS Coordinator to join our healthcare team. In this pivotal role, you will oversee the comprehensive assessment and documentation of patient care needs within skilled nursing, rehabilitation, or long-term care settings. Your expertise will ensure accurate and timely completion of Minimum Data Set (MDS) assessments, which are essential for quality patient care, regulatory compliance, and reimbursement processes. This position offers an exciting opportunity to make a meaningful impact on patient outcomes while working within a dynamic healthcare environment that values precision, collaboration, and excellence.Duties<ul><li>Coordinate and complete comprehensive MDS assessments for residents or patients in accordance with federal and state regulations, including CMS guidelines.</li><li>Collaborate with interdisciplinary teams&mdash;nursing staff, physicians, social workers, and therapy providers&mdash;to gather accurate clinical information necessary for MDS documentation.</li><li>Review medical records, clinical documentation, and patient histories to ensure completeness and accuracy of data used in assessments.</li><li>Ensure compliance with all applicable regulations such as HIPAA, NCQA standards, and Medicare requirements related to MDS and patient documentation.</li><li>Utilize electronic health record (EHR) systems like Epic, Cerner, Athenahealth, or eClinicalWorks to input data efficiently and accurately.</li><li>Conduct ongoing audits of medical records and documentation to maintain high standards of clinical documentation improvement (CDI).</li><li>Assist in discharge planning by coordinating with case management teams to facilitate smooth transitions of care.</li><li>Stay current with updates in ICD-10/ICD-9 coding, CPT coding, DRG classifications, and other medical coding standards relevant to MDS assessments.</li><li>Participate in utilization review and utilization management activities to optimize patient care pathways while ensuring regulatory compliance.</li><li>Provide training and support to staff on MDS processes, documentation requirements, and regulatory updates.</li><li>Maintain meticulous records of assessments, audits, and compliance activities related to MDS responsibilities.</li></ul>Skills<ul><li>Extensive knowledge of MDS processes, including completion timelines and regulatory requirements.</li><li>Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.</li><li>Strong understanding of managed care policies and Medicare/Medicaid reimbursement frameworks.</li><li>Critical care experience in settings such as ICU or Level I/II trauma centers is highly desirable.</li><li>Background in hospital or outpatient clinic environments with familiarity in acute care settings like PICU or emergency medicine is advantageous.</li><li>Experience with medical coding including ICD-10/ICD-9 coding standards, CPT codes, DRG classifications, and documentation review techniques.</li><li>Knowledge of physiology, anatomy, nursing practices, medical terminology, and healthcare regulations including HIPAA compliance.</li><li>Ability to interpret complex medical records accurately and perform detailed documentation review.</li><li>Excellent organizational skills with the ability to manage multiple priorities efficiently under tight deadlines.</li><li>Strong communication skills for effective collaboration across multidisciplinary teams.</li><li>Familiarity with health information management systems such as Meditech or similar platforms is a plus.</li><li>Prior experience in discharge planning, hospice care coordination or home care settings is beneficial.</li><li>Demonstrated commitment to maintaining high standards aligned with NCQA accreditation standards. Registered Nurse / MDS Coordinator

Responsibilities: - Provide direct patient care, including assessing, planning, implementing, and evaluating patient care plans - Administer medications and treatments as prescribed by the physician - Monitor and report changes in patient symptoms or behavior - Collaborate with interdisciplinary healthcare team members to provide comprehensive care - Educate patients and their families on disease processes, treatment plans, and self-care techniques - Assist in medical procedures such as phlebotomy and wound care - Maintain accurate and detailed medical records and documentationQualifications: - Valid Registered Nurse (RN) license in the state of practice - Proven experience in a primary care or hospital setting - Knowledge of pain management techniques and medication administration - Ability to lift and move patients safely when necessary - Familiarity with medical imaging and diagnostic procedures - Proficient in using electronic medical record systems - Excellent communication and interpersonal skillsNote: This job description is not intended to be all-inclusive.
