We are seeking a highly motivated and detail-oriented MDS Coordinator to join our healthcare team. Duties
- Coordinate and complete comprehensive MDS assessments for residents or patients in accordance with federal and state regulations, including CMS guidelines.
- Collaborate with interdisciplinary teams—nursing staff, physicians, social workers, and therapy providers—to gather accurate clinical information necessary for MDS documentation.
- Review medical records, clinical documentation, and patient histories to ensure completeness and accuracy of data used in assessments.
- Ensure compliance with all applicable regulations such as HIPAA, NCQA standards, and Medicare requirements related to MDS and patient documentation.
- Utilize electronic health record (EHR) systems like Epic, Cerner, Athenahealth, or eClinicalWorks to input data efficiently and accurately.
- Conduct ongoing audits of medical records and documentation to maintain high standards of clinical documentation improvement (CDI).
- Assist in discharge planning by coordinating with case management teams to facilitate smooth transitions of care.
- Stay current with updates in ICD-10/ICD-9 coding, CPT coding, DRG classifications, and other medical coding standards relevant to MDS assessments.
- Participate in utilization review and utilization management activities to optimize patient care pathways while ensuring regulatory compliance.
- Provide training and support to staff on MDS processes, documentation requirements, and regulatory updates.
- Maintain meticulous records of assessments, audits, and compliance activities related to MDS responsibilities.
Skills
- Extensive knowledge of MDS processes, including completion timelines and regulatory requirements.
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Strong understanding of managed care policies and Medicare/Medicaid reimbursement frameworks.
- Critical care experience in settings such as ICU or Level I/II trauma centers is highly desirable.
- Background in hospital or outpatient clinic environments with familiarity in acute care settings like PICU or emergency medicine is advantageous.
- Experience with medical coding including ICD-10/ICD-9 coding standards, CPT codes, DRG classifications, and documentation review techniques.
- Knowledge of physiology, anatomy, nursing practices, medical terminology, and healthcare regulations including HIPAA compliance.
- Ability to interpret complex medical records accurately and perform detailed documentation review.
- Excellent organizational skills with the ability to manage multiple priorities efficiently under tight deadlines.
- Strong communication skills for effective collaboration across multidisciplinary teams.
- Familiarity with health information management systems such as Meditech or similar platforms is a plus.
- Prior experience in discharge planning, hospice care coordination or home care settings is beneficial.
