This position offers an opportunity to work closely with clients and healthcare professionals, delivering high-quality home health services that enhance quality of life.Duties<ul><li>Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support</li><li>Monitor and observe patient conditions diligently, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems</li><li>Administer medications and manage medical documentation in compliance with HIPAA regulations to ensure patient confidentiality and safety</li><li>Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature checks regularly to track health trends</li><li>Support clients with specialized care needs including Alzheimer&rsquo;s care, dementia care, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities</li><li>Operate medical equipment such as Hoyer lifts for safe patient transfers and assist with medical procedures like medication administration under supervision</li><li>Prepare nutritious meals and assist with light housekeeping tasks like cleaning and laundry to maintain a clean and safe living environment</li><li>Use EMR/EHR systems efficiently for documentation of patient care activities and medical records management</li><li>Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation</li></ul>Skills<ul><li>Strong knowledge of medical terminology, patient observation techniques, and dementia or Alzheimer&rsquo;s care practices</li><li>Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience preferred</li><li>Ability to assist with complex caregiving tasks such as Hoyer lift operation, catheter care, medication administration, and vital signs monitoring</li><li>Excellent communication skills for working effectively with patients, families, and healthcare teams while maintaining HIPAA compliance</li><li>Compassionate attitude combined with the ability to handle sensitive situations involving hospice or memory care patients</li><li>Proficiency in using EMR/EHR systems for accurate medical documentation and record keeping</li><li>Physical stamina to perform caregiving duties that may include lifting, cleaning, meal preparation, and assisting with mobility needs</li><li>Valid CPR certification required; additional certifications in hospice or dementia care are a plus</li><li>Flexibility to work live-in employment arrangements or scheduled shifts that meet client needs</li></ul>Join us in providing exceptional home health services that empower individuals to live safely and comfortably at home.
