Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and observe patient conditions diligently, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Administer medications and manage medical documentation in compliance with HIPAA regulations to ensure patient confidentiality and safety
- Perform vital signs assessments such as blood pressure, pulse, respiration, and temperature checks regularly to track health trends
- Support clients with specialized care needs including Alzheimer’s care, dementia care, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities
- Operate medical equipment such as Hoyer lifts for safe patient transfers and assist with medical procedures like medication administration under supervision
- Prepare nutritious meals and assist with light housekeeping tasks like cleaning and laundry to maintain a clean and safe living environment
- Use EMR/EHR systems efficiently for documentation of patient care activities and medical records management
- Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation
Skills
- Strong knowledge of medical terminology, patient observation techniques, and dementia or Alzheimer’s care practices
- Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience preferred
- Ability to assist with complex caregiving tasks such as Hoyer lift operation, catheter care, medication administration, and vital signs monitoring
- Excellent communication skills for working effectively with patients, families, and healthcare teams while maintaining HIPAA compliance
- Compassionate attitude combined with the ability to handle sensitive situations involving hospice or memory care patients
- Proficiency in using EMR/EHR systems for accurate medical documentation and record keeping
- Physical stamina to perform caregiving duties that may include lifting, cleaning, meal preparation, and assisting with mobility needs
- Valid CPR certification required; additional certifications in hospice or dementia care are a plus
- Flexibility to work live-in employment arrangements or scheduled shifts that meet client needs
