The ideal candidate will possess a strong commitment to patient well-being, with experience in assisting individuals with diverse needs, including seniors, those with developmental disabilities, and individuals requiring memory or dementia care. This role offers an opportunity to make a meaningful difference in clients' lives by delivering personalized, respectful, and professional care within their homes. Candidates should be detail-oriented, empathetic, and capable of working independently while adhering to all safety and privacy standards.MUST HAVE EXPERIENCE WITH TOTAL CARE/ BED BOUND CLIENTS(Hoyer Lift, Bed Bath, Repositioning, Incontinence, etc.)Responsibilities<ul><li>Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.</li><li>Monitor and observe patients&rsquo; health status, documenting vital signs and changes in condition accurately using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems.</li><li>Administer medications as prescribed, ensuring proper documentation in compliance with HIPAA regulations.</li><li>Support clients with specialized needs such as catheter care, Alzheimer&rsquo;s care, dementia care, hospice support, and memory care.</li><li>Utilize equipment such as Hoyer lifts for safe patient transfers and assist with medical procedures under supervision when necessary.</li><li>Prepare nutritious meals aligned with dietary restrictions and assist with meal planning and preparation to promote health and well-being.</li><li>Maintain a clean and safe environment by performing light housekeeping tasks including laundry and general cleaning.</li><li>Communicate effectively with clients, family members, and healthcare teams to ensure comprehensive care delivery.</li></ul>Qualifications<ul><li>Valid CNA certification or relevant caregiving credentials; experience as a PCA is highly valued.</li><li>Prior experience working in assisted living facilities, nursing homes, or home care environments.</li><li>Experience working with individuals with disabilities or developmental challenges preferred.</li><li>CPR certification required; additional training in hospice or Alzheimer&rsquo;s/dementia care is advantageous.</li><li>Ability to lift or transfer patients using equipment such as Hoyer lifts safely and effectively.</li><li>Strong interpersonal skills combined with patience, compassion, and professionalism in all interactions.
