Overview
We are seeking compassionate and dedicated PCA (Personal Care Assistant) / CNA (Certified Nursing Assistant) to provide exceptional home care services in the Stone Mountain area. The ideal candidate will possess a strong commitment to patient well-being, with experience in assisting individuals with diverse needs, including seniors, those with developmental disabilities, and individuals requiring memory or dementia care. This role offers an opportunity to make a meaningful difference in clients' lives by delivering personalized, respectful, and professional care within their homes. Candidates should be detail-oriented, empathetic, and capable of working independently while adhering to all safety and privacy standards.

MUST HAVE EXPERIENCE WITH TOTAL CARE/ BED BOUND CLIENTS

(Hoyer Lift, Bed Bath, Repositioning, Incontinence, etc.)

Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
• Monitor and observe patients’ health status, documenting vital signs and changes in condition accurately using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems.
• Administer medications as prescribed, ensuring proper documentation in compliance with HIPAA regulations.
• Support clients with specialized needs such as catheter care, Alzheimer’s care, dementia care, hospice support, and memory care.
• Utilize equipment such as Hoyer lifts for safe patient transfers and assist with medical procedures under supervision when necessary.
• Prepare nutritious meals aligned with dietary restrictions and assist with meal planning and preparation to promote health and well-being.
• Maintain a clean and safe environment by performing light housekeeping tasks including laundry and general cleaning.
• Communicate effectively with clients, family members, and healthcare teams to ensure comprehensive care delivery.

Qualifications

• Valid CNA certification or relevant caregiving credentials; experience as a PCA is highly valued.
• Prior experience working in assisted living facilities, nursing homes, or home care environments.
• Experience working with individuals with disabilities or developmental challenges preferred.
• CPR certification required; additional training in hospice or Alzheimer’s/dementia care is advantageous.
• Ability to lift or transfer patients using equipment such as Hoyer lifts safely and effectively.
