Family Health Matters Community Health Center is seeking a Quality Improvement &amp; Data Analytics Manager to lead organizational quality initiatives, oversee performance reporting, and support regulatory compliance and data governance.This cross-functional leadership role serves as the central point of coordination for quality improvement, data reporting accuracy, and performance monitoring across clinical and operational programs. The position plays a key role in supporting HRSA requirements, value-based care performance, and population health outcomes.What You&rsquo;ll DoQuality Improvement &amp; Performance Management<ul><li>Lead development and implementation of the organization&rsquo;s Quality Improvement Plan</li><li>Monitor performance against clinical and operational targets</li><li>Identify performance gaps and lead corrective action planning</li><li>Support and coordinate Quality Improvement Committee activities</li><li>Develop presentations and reports for leadership teams</li></ul>Data Analytics &amp; Reporting<ul><li>Lead coordination of UDS reporting and submission readiness</li><li>Develop dashboards and performance scorecards</li><li>Extract, validate, and analyze data from EHR systems</li><li>Translate data into actionable insights for leadership and care teams</li><li>Ensure reporting accuracy and data integrity</li></ul>Value-Based Care &amp; Network Performance<ul><li>Monitor quality performance across network partners</li><li>Track care gap data and quality metrics</li><li>Support supplemental data submissions (e.g., Cozeva or similar platforms)</li><li>Identify opportunities to improve performance and outcomes</li></ul>EHR Data Governance &amp; System Support<ul><li>Lead governance of EHR templates, visit types, and data mappings</li><li>Ensure alignment between workflows and reporting requirements</li><li>Collaborate with IT and clinical teams to improve data systems</li><li>Support EHR-related enhancements and reporting reliability</li></ul>Compliance &amp; Risk Management<ul><li>Support HRSA compliance and audit readiness</li><li>Maintain risk management and incident tracking processes</li><li>Support regulatory programs such as PCMH and Title X</li><li>Monitor grievances and identify trends for improvement</li></ul>What We&rsquo;re Looking ForRequired<ul><li>Bachelor&rsquo;s degree in Public Health, Healthcare Administration, Health Informatics, Data Analytics, or related field</li><li>Minimum 2 years of experience in:</li><li>Healthcare quality improvement</li><li>Data analytics</li><li>Population health</li><li>Program evaluation</li><li>Experience working with healthcare data and EHR systems</li><li>Strong ability to analyze data and present findings clearly</li></ul>Preferred<ul><li>Experience working in an FQHC or community health center</li><li>Familiarity with HRSA and UDS reporting</li><li>Experience supporting value-based care programs</li><li>Certification such as CPHQ or Lean/Six Sigma</li></ul>Why Join Us<ul><li>Mission-driven community health center</li><li>Opportunity to lead meaningful quality and data initiatives</li><li>Collaborative leadership environment</li><li>Work that directly improves care for underserved communities</li><li>Growth opportunities in population health and healthcare analytics</li></ul>ScheduleFull-Time | Alternative Work Week (4/10 Schedule) Typical schedule consists of four 10-hour shifts per week.View Full Job DescriptionTo review the complete job description, including full responsibilities and qualifications, please visit: www.fhm-chc.org/careersApply TodayIf you are passionate about healthcare quality, data-driven decision-making, and improving outcomes for underserved communities, we encourage you to apply.Pay: $72,000.00 - $80,000.00 per yearBenefits:<ul><li>Dental insurance</li><li>Health insurance</li><li>Life insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Application Question(s):<ul><li>Please describe your experience working with UDS reporting or healthcare quality metrics.</li><li>What EHR systems have you used, and what type of data reporting did you perform?</li><li>Describe your experience developing dashboards or performance reports.</li><li>Do you have experience working in an FQHC or community health center?
