Quality Improvement & Data Analytics Manager

Family Health Matters Community Health Center is seeking a Quality Improvement & Data Analytics Manager to lead organizational quality initiatives, oversee performance reporting, and support regulatory compliance and data governance.

This cross-functional leadership role serves as the central point of coordination for quality improvement, data reporting accuracy, and performance monitoring across clinical and operational programs. The position plays a key role in supporting HRSA requirements, value-based care performance, and population health outcomes.

What You’ll Do

Quality Improvement & Performance Management

  • Lead development and implementation of the organization’s Quality Improvement Plan
  • Monitor performance against clinical and operational targets
  • Identify performance gaps and lead corrective action planning
  • Support and coordinate Quality Improvement Committee activities
  • Develop presentations and reports for leadership teams

Data Analytics & Reporting

  • Lead coordination of UDS reporting and submission readiness
  • Develop dashboards and performance scorecards
  • Extract, validate, and analyze data from EHR systems
  • Translate data into actionable insights for leadership and care teams
  • Ensure reporting accuracy and data integrity

Value-Based Care & Network Performance

  • Monitor quality performance across network partners
  • Track care gap data and quality metrics
  • Support supplemental data submissions (e.g., Cozeva or similar platforms)
  • Identify opportunities to improve performance and outcomes

EHR Data Governance & System Support

  • Lead governance of EHR templates, visit types, and data mappings
  • Ensure alignment between workflows and reporting requirements
  • Collaborate with IT and clinical teams to improve data systems
  • Support EHR-related enhancements and reporting reliability

Compliance & Risk Management

  • Support HRSA compliance and audit readiness
  • Maintain risk management and incident tracking processes
  • Support regulatory programs such as PCMH and Title X
  • Monitor grievances and identify trends for improvement

What We’re Looking For

Required

  • Bachelor’s degree in Public Health, Healthcare Administration, Health Informatics, Data Analytics, or related field
  • Minimum 2 years of experience in:
  • Healthcare quality improvement
  • Data analytics
  • Population health
  • Program evaluation
  • Experience working with healthcare data and EHR systems
  • Strong ability to analyze data and present findings clearly

Preferred

  • Experience working in an FQHC or community health center
  • Familiarity with HRSA and UDS reporting
  • Experience supporting value-based care programs
  • Certification such as CPHQ or Lean/Six Sigma

Full-Time | Alternative Work Week (4/10 Schedule)
