We are seeking a <b>Licensed Intake Coordinator</b> to support our Houston market by leading the intake and assessment process for new clients.</p><h3 class="jobSectionHeader"><b>
Position Summary</b></h3><p>
The Intake Coordinator is a <b>mid-level clinical role</b> responsible for managing the full intake process&mdash;from initial referral to treatment planning. This position requires strong clinical assessment skills, knowledge of DSM-5 diagnostic criteria, and the ability to ensure medical necessity and regulatory compliance.</p><p>
This role plays a critical part in ensuring timely access to care, completing comprehensive assessments, and coordinating services with both internal teams and external providers.</p><h3 class="jobSectionHeader"><b>
Key Responsibilities</b></h3><h4 class="jobSectionHeader"><b>
Intake &amp; Clinical Assessment</b></h4><ul><li>
Contact new referrals promptly to initiate services and reduce wait times.</li><li>
Conduct comprehensive diagnostic assessments, including mental status evaluations, risk assessments, and review of clinical history.</li><li>
Formulate accurate diagnoses using DSM-5 criteria and determine medical necessity for services.</li><li>
Identify appropriate levels of care and make referrals when needed.</li></ul><h4 class="jobSectionHeader"><b>
Treatment Planning &amp; Documentation</b></h4><ul><li>
Develop individualized, person-centered treatment plans within required timelines.</li><li>
Ensure all documentation is accurate, complete, and compliant with state, federal, and organizational standards.</li><li>
Maintain timely entry of clinical documentation, including assessments and progress notes.</li><li>
Submit required documentation for clinical review and approval as appropriate.</li></ul><h4 class="jobSectionHeader"><b>
Care Coordination</b></h4><ul><li>
Collaborate with families, support systems, and external providers (e.g., schools, primary care providers, community agencies) to support continuity of care.</li><li>
Coordinate appointments and referrals, including psychiatric or medication management services.</li><li>
Support a smooth transition from intake to ongoing services.</li></ul><h4 class="jobSectionHeader"><b>
Operational Responsibilities</b></h4><ul><li>
Track referrals and maintain accurate intake records.</li><li>
Ensure client demographic and service data are up to date in the electronic health record.</li><li>
Participate in team meetings, trainings, and quality improvement initiatives.</li><li>
Adhere to all organizational policies, procedures, and compliance standards.</li></ul><h4 class="jobSectionHeader"><b>
Additional Clinical Duties</b></h4><ul><li>
May provide limited direct services (e.g., therapy, crisis intervention, or skills-based services) within scope of licensure as needed.</li></ul><h3 class="jobSectionHeader"><b>
Minimum Qualifications</b></h3><p><b>
Required:</b></p><ul><li>
Master&rsquo;s degree in Social Work, Counseling, Psychology, or a related behavioral health field</li><li>
Current independent clinical licensure (LPC, LCSW, LMFT, or higher)</li><li>
Minimum 2&ndash;3 years of clinical experience, including assessment and diagnosis</li><li>
Bilingual (English/Spanish) required</li></ul><p><b>
Preferred:</b></p><ul><li>
Experience in community-based or Medicaid-funded behavioral health services</li><li>
Familiarity with trauma-informed care and evidence-based assessment practices</li></ul><h3 class="jobSectionHeader"><b>
