First Light is looking to fill a case manager position in its Permanent Supportive Housing Program. Qualified candidates must meet the following requirements:&middot; Bachelor&rsquo;s degree in human service field of study, degree in social work preferred&middot; Sensitivity to homelessness, mental illness and substance abuse&middot; Must be a self-initiator, work independent as needed and be able to problem solve various life issues&middot; Have reliable transportation, valid driver&rsquo;s license and automobile insurance&middot; Ability to use a computer, have good organizational, communication, and interpersonal skills&middot; Able to be flexible and appropriately handle stressful situationsDuties and Responsibilities&middot; The ability to network with landlords and develop relationships with community service providers&middot; Meet with participants in their homes, at least monthly and more often as appropriate, to assess needs specifically related to housing and their well being&middot; Ability to work with clients to develop solutions to ensure best support for the success&middot; Conduct comprehensive assessments and help tenants develop action plans to achieve goals&middot; Complete home visits and assess client safety, mental and medical health, and environmental health<ul><li>Design individual case plans to address specific needs on each client and evaluate and adjust case plans as needed</li><li>Provide direct services including case management services assessing individual needs</li><li>Assist clients by teaching participants how to access resources such as health care, food assistance and job training opportunities in Birmingham</li><li>Provide transportation as needed</li><li>Build professional relationships with clients, landlords, providers, and community partners</li><li>Document all client interaction and maintain accurate paperwork/reports</li><li>Perform annual re-certifications and inspections for each client and accurately update files as necessary</li></ul>Job Type: Full-timeBenefits:<ul><li>401(k)</li><li>401(k) matching</li><li>Dental insurance</li><li>Flexible schedule</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Schedule:<ul><li>8 hour shift</li><li>Monday to Friday</li><li>On call</li></ul>Work Location: In person
