First Light is looking to fill a case manager position in its Permanent Supportive Housing Program. Qualified candidates must meet the following requirements:
· Bachelor’s degree in human service field of study, degree in social work preferred
· Sensitivity to homelessness, mental illness and substance abuse
· Must be a self-initiator, work independent as needed and be able to problem solve various life issues
· Have reliable transportation, valid driver’s license and automobile insurance
· Ability to use a computer, have good organizational, communication, and interpersonal skills
· Able to be flexible and appropriately handle stressful situations
Duties and Responsibilities
· The ability to network with landlords and develop relationships with community service providers
· Meet with participants in their homes, at least monthly and more often as appropriate, to assess needs specifically related to housing and their well being
· Ability to work with clients to develop solutions to ensure best support for the success
· Conduct comprehensive assessments and help tenants develop action plans to achieve goals
· Complete home visits and assess client safety, mental and medical health, and environmental health
