Position Title: Medical Social Worker/Case ManagerReports To: Director of Social ServicesPosition Purposes: The Medical Social Worker/Case Manager provides affirming, respectful, and supportive care to individuals from diverse backgrounds and lived experiences. The Medical Social Worker/Case Manager is responsible for conducting biopsychosocial assessments, developing case plans, providing case management services, and working as part of the interdisciplinary health team for clients of Five Horizons Health Services (FHHS).Essential Functions:&middot; Provide direct social work services to a caseload of clients at different levels of care as dictated by their care plans to facilitate client retention in medical care&middot; Complete biannual psychosocial assessments and case plans&middot; Facilitate support groups for HIV clients&middot; Complete EFA (Emergency Financial Assistance)&middot; Collaborate with client&rsquo;s medical team to identify and address medical needs related to accessing care and treatment&middot; Participate in multidisciplinary health teams and provide feedback from the social work perspective&middot; Maintain a strong knowledge of available services to include Ryan White HIV / AIDS Program (RWHAP) Part B, AIDS Drug Assistance Program (ADAP), and Alabama Insurance Assistance Program (AIAP)&middot; Demonstrate a skilled knowledge of engagement, assessment, case planning, and monitoring techniques to maintain the helping relationship between the client and FHHS&middot; Promote client participation in the healthcare continuum through face-to-face encounters, telephone interactions, text messaging, and any other engagement techniques as needed&middot; Enhance clients&rsquo; understanding of HIV by demonstrating and relaying a skilled knowledge of HIV / AIDS, Hepatitis C, Pre-Exposure Prophylaxis (PrEP) and their effects on individuals and families&middot; Maintain required client records, document services provided, scan client documents into EHR, and complete other required paperwork in a timely manner&middot; Maintain compliance with HIPAA and state guidelines as well as FHHS policy and procedures&middot; Perform any other department / agency-related duties or special projects as directed by supervisor&middot; As with all other positions, during emergencies, the Medical Social Worker is expected to fill in at any position within the agency for which they can performEducation/Professional:&middot; Bachelor&rsquo;s or Master&rsquo;s Degree in Social Work&middot; Licensure or license-eligible at the appropriate level through the Alabama State Board of Social Work Examiners&middot; Must possess a strong ability to interact with diverse groups and cultures&middot; Strong organizational and time management skillsPhysical Requirements: Frequent sitting, some standing / walking, and occasionally lifting supplies / equipment up to 50 lbs. Occasional reaching, bending, stooping, kneeling and crouching. This position requires an individual who is mentally alert and able to effectively communicate verbally and in writing. Position Title: EFA Program Assistant IIReports To: Director of EFA ProgramPosition Purposes: The EFA Program Assistant will provide support to the Director of the EFA Program through direct communication with RWPB providers and case managers, assisting with ongoing case manager training, and supporting the entire lifecycle of the EFA application process.Essential Functions:&middot; Provide support to case managers at RWPB provider organizations for the EFA application process&middot; Assist in hosting mandatory in-person training meeting in Mississippi for all RWPB EFA providers&middot; Provide technical assistance for EFA application portal&middot; Support the full life cycle of the EFA application process from start to finish, ensuring that applications are complete, valid, and include all necessary documentation&middot; Address any deficiencies in EFA applications and consult with relevant case managers&middot; Provide education and support on the EFA policies and procedures&middot; Coordinate with IT to ensure all relevant backend website needs are addressed in a timely manner&middot; Assist with data collection, reporting, and required site visits to maintain compliance with MSDH and HRSA&middot; Remain up to date on MSDH, HRSA, and RWPB program policies and procedures&middot; Commit 8-16 hours of time weekly to travel and on-site training to staff and providers, ensuring consistent support and effective implementation of processes.&middot; Ensure accurate collection of data and secure storage of applications, including all required documentation in compliance with RWPB policies for auditing purposes&middot; Coordinate with Finance to ensure all approved applications are accounted for payment processing&middot; Process payments by calling vendors to confirm, verify and resolve balances&middot; Travel may be required to vendor locations for the purpose of processing payments, purchasing vouchers, delivering payments, and mailing payment-related documents.&middot; Other duties as assignedEducation/Professional:&middot; High School Diploma or Equivalent Required; bachelor&rsquo;s degree or Higher Preferred (particularly in fields such as Social Work, Psychology, Business Administration, etc.)Physical Requirements: This position requires an individual who is self-motivated and able to independently complete assigned responsibilities.
