</b></p><p>We are seeking an experienced <b>Home Health Marketing Liaison </b>with a strong background in the <b>Los Angeles healthcare market </b>and proven success in cardiology-related referral development. This is an excellent opportunity for a healthcare sales professional passionate about building relationships and driving growth in home health services.
</p><p></p><p><b>Key Responsibilities
</b></p><ul><li>Develop and maintain referral relationships with cardiologists, hospitals, case managers, discharge planners, and physician offices across the LA market
</li><li>Promote home health care services, emphasizing cardiac care and chronic disease management programs
</li><li>Consistently achieve monthly and quarterly admissions and referral targets
</li><li>Educate referral sources on clinical outcomes, patient benefits, and regulatory compliance
</li><li>Monitor market trends, competitor activity, and payer mix
</li><li>Collaborate with clinical and operations teams for seamless patient transitions
</li><li>Document all sales activities and referral data in CRM systems
</li></ul><p></p><p><b>Qualifications
</b></p><ul><li>3&ndash;5 years of home health or healthcare marketing experience in the Los Angeles market (required)
</li><li>Proven success working with cardiology practices or cardiac-related services (strongly preferred)
</li><li>Established network of referral sources in LA and surrounding areas
</li><li>Bachelor&rsquo;s degree preferred (Healthcare or Business-related field a plus)
</li><li>Excellent communication, presentation, and negotiation skills
</li><li>Self-motivated, organized, and results-driven
</li><li>Compassionate, patient-first mindset
</li><li>Valid driver&rsquo;s license and reliable transportation
