Job Overview Heartsong Hospice is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our hospice care team. In this vital role, you will provide personalized, respectful, and high-quality care to patients in their final stages of life, ensuring comfort and dignity. This position offers an opportunity to make a meaningful difference while working in a dynamic healthcare setting that values professionalism and teamwork.Duties<ul><li>Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support.</li><li>Provide specialized hospice care that emphasizes comfort measures, pain management, and emotional support for terminally ill patients.</li><li>Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems.</li><li>Assist with transfers and positioning to prevent bedsores and promote patient comfort in accordance with safety protocols.</li><li>Engage with patients experiencing dementia or memory care needs, applying techniques suitable for Alzheimer's and other cognitive impairments.</li></ul>Qualifications<ul><li>Valid certification as a Certified Nursing Assistant (CNA) with current state licensure.</li><li>Dependable transportation</li><li>Experience working in hospice or palliative care environments is highly preferred.</li><li>Demonstrated knowledge of medical terminology, anatomy, physiology, and aseptic techniques.</li><li>Experience with EMR/EHR systems for accurate documentation.</li><li>Prior work in long-term care facilities such as nursing homes, assisted living, or home &amp; community care settings preferred.</li><li>Strong caregiving skills with an emphasis on geriatrics, behavioral health, dementia care, and working with individuals with disabilities or developmental challenges.</li><li>Excellent communication skills combined with the physical stamina required for patient service tasks such as lifting and transferring using Hoyer lifts or other assistive devices.</li><li>Join our team if you&rsquo;re passionate about providing compassionate hospice care that truly makes a difference!
