<div>Stoneegate is seeking an experienced MDS/PPS COORDINATOR to join our team!<ul><li>
LVN or RN</li></ul>
MDS COORDINATOR
In this role, you will:<ul><li>
Coordinate Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments.</li><li>
Ensure Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS).</li><li>
Monitor Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization.</li><li>
Utilize MDS Software: Maintain expertise in software systems to ensure correct data entry and reporting.</li><li>
Lead Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.</li><li>
Update Care Plans: Adjust and revise care plans as needed based on current assessments and resident status.</li><li>
Provide Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement.</li><li>
Submit MDS Data: Ensure timely and accurate transmission of MDS assessments per reporting standards.</li><li>
Stay Current: Attend professional development sessions to keep up with industry standards and best practices.</li><li>
Oversee Ancillary Data: Collect and report ancillary services to corporate billing for accurate processing.</li></ul>
Knowledge / Experience:<ul><li>
Must have 2 years of long term care experience</li><li>
Must have MDS / TILE experience</li><li>
Must be fluent in MDS 3.0</li></ul>
Licensure / Certifications:<ul><li>
Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.</li></ul>
